Or are you expecting future mods to search through thousands of pages of mod actions and hundreds of forum topics for every single report just to see if some lazy mod didn't document something?

There is literally a vio for derailing threads. And it is known as forum spam vio. Make use of it to moderate forums. Make use of hateful comments vio. There is no need to punish people unfairly just because they did something wrong in your eyes.
