This happens when more than one people create a new thread at exactly the same time. Not even a nano second apart. Then it throws an error and all threads on the server will show this error message.

We have tested this on my website. We created a bot to create the same thread at the same time.

So I am assuming that someone tried to create a forum thread or a game or a report with exactly the same details and clicked at exactly the same time.
